Why Is Bone Grafting Necessary?

The success of a dental implant, both in aesthetics and support for restorations, largely depends on the volume of jawbone in the implant site.

Jawbone volume can diminish for several reasons, including periodontal disease, trauma, and infections. Bone grafting increases bone volume and improves the shape of the jaw ridge, facilitating successful implant placement. The procedure is performed in-office using various bone graft materials, such as:

  • Autogenous bone (from the patient’s own body)
  • Animal-derived bone (e.g., bovine or porcine)
  • Synthetic materials

Why Is Sinus Lifting Necessary?

The maxillary sinuses are two air-filled cavities located on either side of the facial bones. As we age, these sinuses expand and may encroach on the roots of the upper molars. When upper molars are lost, the sinuses can further enlarge into the space where the teeth used to be, causing significant bone loss.

Without a sinus lift and bone graft in these areas, implant placement is not feasible due to insufficient bone volume to support the implant.

Bone Grafting Techniques

1. Block Bone Grafting

This method involves harvesting a small block of bone from another area of the patient’s body, typically the chin or near the wisdom teeth, and placing it in the implant site.

  • This technique is used when the bone volume in the implant area is severely reduced, often due to long-term tooth loss.
  • Implant placement typically occurs six months after the graft has fused with the recipient site.

2. Particulate Bone Grafting

In cases of minor bone loss, implant placement and bone grafting can be done simultaneously. The areas of the implant not covered by natural bone are filled with particulate graft material, which can include:

  • Bovine or porcine bone
  • Synthetic materials
  • The patient’s own bone
    This method allows for quicker treatment times and is ideal for moderate bone deficiencies.

3. Ridge Preservation Grafting

The alveolar ridge, the bone structure supporting the teeth, rapidly deteriorates after tooth extraction.

  • Ridge preservation involves grafting bone into the tooth socket immediately after extraction or within 1–2 weeks to prevent bone loss.
  • Teeth are removed gently using piezotomes instead of traditional forceps to minimize trauma.
    This technique facilitates future implant placement by maintaining the natural bone structure.
